Stopping the Unstoppable

Position analysis from several moves back in my current game.

As White, I was feeling pretty cocky. After a series of trades, my opponent dropped one of his pawns and allowed me to gain a passed pawn. With a strong pawn chain, I was feeling pretty good about myself when I first moved into this position, but my anticipation quickly turned to fear when I looked deeper into it.

Black to move:

What did I see to make me so afraid? Was that fear justified?

(Note: Although this is a current game, this position was established seven moves prior to the current move. Black did not make what I believed to be his optimal move here, and thus analysis of this particular position will not affect my game.)
